Community Impact Grant Application 2020
Applications for Community Impact Grant funding is now open!
Community Impact Grants fund programs improving education, financial stability or health for the residents of Wapello County. Grant applications are due May 18, 2020. Funding, if awarded, starts July 1, 2020 and ends June 30, 2021.
Indicators and Outcomes
United Way of Wapello County has four focus areas: childhood success, youth success, financial stability, and health. All grantees must report the number of people served, as well as minimum one indicator and one outcome in their chosen focus area.
Childhood Success Indicators
Number of children attending QRS-rated early childhood programs.
Number of children attending 85% of available early education days.
Percentage of children attending 85% of available early childhood days.
Number of parents participating in parent involvement or education opportunities.
Percentage of parents who participate in at least 3 parent involvement opportunities.
Childhood Success Outcomes
Percentage of children (age 0-2) who achieve age-appropriate milestones in physical development.
Percentage of children (age 0-5) who achieve age-appropriate cognitive milestones in math, science, literacy and creative expression.
Percentage of children served (K-3) reading at grade level.
Percentage of children maintaining or improving math and literacy skills through out of school time programs.
Youth Success Indicators
Number of youth or parents who report improved life skills.
Number of youth reporting improved behavior or school attendance.
Number of youth who improve academic performance.
Number of participants reporting higher aspirations for graduating high school and/or enrolling in a college or credential program.
Youth Success Outcomes
Percentage of youth served who graduate high school on time.
Percentage of youth who served who enroll in port-secondary education.
Percentage of youth served (age 15-24) who gain employment.
Financial Stability Indicators
Number of people who apply for or pursue claims for benefits or income supports.
Number of people who address credit problems by obtaining a credit report, repairing credit, reducing debt or establishing credit.
Number of participants who increase their knowledge in financial literacy, money management and consumer safety.
Number of people with increased access to food resources.
Number of people who participate in employment-related skill building.
Number of people receiving housing services.
Financial Stability Outcomes
Percentage of people served who improve their credit score.
Percentage of people served with increased knowledge or skills related to cost-effective food choices.
Percentage of people served who gain employment or increase wages.
Percentage of people served who obtain or maintain stable, safe housing.
Percentage of individuals served who increase their disposable income by accessing benefits or reducing costs.
Health Indicators
Number of patient visits.
Number of patients who are low income.
Number of patients who are uninsured or under-insured.
Number of patients with limited English proficiency.
Number of people accessing healthy food or nutrition programs.
Health Outcomes
Percentage of participants who have reduced risky behaviors, including alcohol, drug or tobacco use.
Percentage of participants who report an increase in healthy behavior or improved nutrition.
Percentage of patients that attend scheduled appointments.
Eligibility
To be eligible for United Way of Wapello County funding, agencies must meet all of the following requirements:
- The organization is an incorporated nonprofit organization under the laws of the state of Iowa and has tax-exempt status under section 501(c)(3) of the Internal Revenue Service code.
- The organization has a Board of Directors that is an active, representative voluntary governing body, with regular meetings and is responsible for developing its mission, determining its strategic direction and providing oversight.
- The organization has bylaws that state the purpose of the organization, provide rules for selection, tenure, number of Board of Directors officers and committees, financial and legal procedures, conflicts of interest, and quorum requirements.
- The Board of Directors is responsible for ensuring that sufficient funds are available for the organization to meet its objectives.

The organization has proof of adequate liability insurance coverage and will indemnify UWWC, its volunteers and staff from any and all liability for any damage or injury caused to any employee, client, patron, agency, visitor or guest of the agency.

The organization can submit an audit (financial review if budget is less than $100,000) prepared by an independent certified public accountant in compliance with generally accepted auditing standards as applied to nonprofit organizations.

The Board of Directors has inspected and approved the annual audit, or review, as well as IRS Form 990.

The organization has and maintains a balanced budget, or has an agreed upon plan for meeting all deficits.

The organization provides funded services in Wapello County, Iowa and its services are culturally sensitive and barrier free.

The organization has identified projects or programs that fall within the scope of one or more of UWWC’s Impact Goals.

The organization has the mechanisms and capacity for regularly reporting measurable, qualitative and quantitative outcomes.

The organization can adhere to the UWWC funding agreement including all documentation and reports by established deadlines.

The organization demonstrates reasonable efficiency in program management and adequacy of resources, both in materials and in personnel (voluntary and paid) to sustain a quality level of service.

The funding request to UWWC does not extend to individuals, civic, religious or political institutions, school fundraisers, capital campaigns or endowments, sponsorship, marketing opportunities or event fundraisers or sports events.
